To save time during your first appointment, you can complete your new patient forms ahead of your visit. These forms are available online as PDFs, and you’ll be able to type your answers in directly. Simply print them and bring them with you to your appointment once they’re filled out.

Payment Options
While we’re not in-network with any specific insurance providers, we are still more than happy to file claims on your behalf and work with you to make the most of your benefits, because we know just how impactful they can be. Please note: for patients who do not use insurance but pay with cash or check, you will receive a 5% discount off of your total bill.
